I declare that I am aware that an aerobatic intro flight, performed in an Extra 300L aircraft, is a high-dynamics sporting flight involving aerobatic manoeuvres (including loops, rolls, inverted flight) and positive and negative G-forces (approximately up to +6G / −4G depending on the package). I acknowledge that the flight may involve temporary discomfort, dizziness, motion sickness and a feeling of disorientation.
I acknowledge that the aeroplane is an aircraft and that taking part in the flight, despite all procedures and safety measures being observed, involves the risk inherent to aviation. I take part in the flight voluntarily and at my own responsibility.
I declare that my health permits me to take part in an aerobatic flight and that I have none of the following contraindications:
I undertake to inform the pilot of any health circumstance that may affect the safety of the flight, including one arising just before take-off.
I declare that on the day of the flight I am sober, not under the influence of alcohol, drugs or medication impairing psychophysical fitness, and that I have not eaten a heavy meal immediately before the flight.
I undertake to strictly follow the instructions of the pilot and airfield staff, to use safety equipment correctly (harness, parachute, helmet, oxygen system if applicable), and to observe the safety rules in force at Radom-Piastów Airfield (EPRP). I acknowledge the participant body-weight limit and that the pilot has the right to refuse or terminate the flight for safety reasons.
